As a Senior Social Worker in the Richmond, Response & Rehabilitation (RRRT) Hospital Discharge Team, you will:
- Lead complex discharge planning
- Oversee hub management

Working across several hospital sites, you will collaborate closely with community Health and Hospital Trust partners to ensure effective, person‑centred discharge pathways. Senior Social Workers play a key role in maintaining high‑quality professional practice.
The use of clear verbal and written communication skills and the ability to represent Adult Social Care confidently in a range of professional forums is essential. You will support the management of the team to deliver a person‑centred service that promotes independence and wellbeing for residents leaving hospital.
Your responsibilities will include:
- Duty, formal supervision, co-ordination of referrals, oversight and clear decision making
- Attending hospital discharge planning meetings
- Being a safeguarding adults manager (SAM) or willing to train as part of this role
- Completing Care Act and Mental Capacity Assessments
You will work a 36‑hour week within a directorate committed to Continuing Professional Development, strengths‑based practice, and forward‑thinking professional standards. You will receive supportive supervision, training, and opportunities to develop your career while contributing to positive outcomes for residents.
Requirements include:
- SWE registration with a recognised Social Work qualification and evidence of ongoing professional development
- Supervision skills - ability to provide reflective, supportive and accountable supervision to staff
- Care Act and MCA competence - strong understanding of Care Act duties, Mental Capacity Assessments and best‑interest decision‑making
- Integrated MDT working - proven ability to work collaboratively with health partners, therapy teams and community services to deliver safe, timely discharge pathways
- High‑quality communication - excellent written and verbal communication with confident, evidence‑based decision‑making
- Strengths‑based practice - commitment to promoting independence, wellbeing and person‑centred outcomes
