Community Specialist Social Workers – Operations, Mental Health, Learning Disability and Safeguarding Teams LB Hammersmith and Fulham £41,964 – £49,638 per annum Town Hall, King Street, London, W6 9JU Hours per week: 36.00 Contract type: Permanent REF: LBHF619404 Community / Operations – The role holder will assist individuals, carers and families to develop support plans to meet the outcomes identified, using a strength-based, asset-based approach and family, friends and community options where appropriate and eligible needs are identified. Mental Health – experience of mental health social work is preferred. There are opportunities to train as Approved Mental Health Professionals, (AMHPs) after a qualifying period. Safeguarding – you will need experience of adult safeguarding, knowledge of relevant legislation and making safeguarding personal. Learning Disability – To provide a professional social work service to adults with a mental, health condition, their families and carers who are ordinary residents of Hammersmith and Fulham in line with the requirements of the Care Act 2014. The post holder will ensure that care and support are person centred and put the people with whom they work in control of their lives. They will work within the legislative frameworks, this will include s.9, s,13 and S.42 of the Care Act 2014, NHS Continuing Healthcare and Community deprivation of liberty will also need to consider. We have a hybrid working model and staff and will be supported to do this with mobile IT equipment and hot desk facilities in all offices across the borough.
