The post holder is responsible for the delivery of nursing care to the practice population. They will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions especially in Diabetes, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Main duties of the job:

  • Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.
  • Implement and evaluate individual treatment plans and management plans for patients with a known long-term condition.
  • Perform appropriate tests for the diagnosing and reviewing of patients with Long Term Conditions (Diabetes).
  • Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care.
  • Deliver opportunistic health promotion using opportunities as identified with individual patients.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions.

Job responsibilities:

  • Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.
  • Implement and evaluate individual treatment plans and management plans for patients with a known long-term condition.
  • Perform appropriate tests for the diagnosing and reviewing of patients with Long Term Conditions, such as Spirometry, Feno Testing.
  • Adopt a holistic approach when assessing and reviewing patients.
  • Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition.
  • Advising patients with respect to their continuing nursing and medical needs.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care.
  • Deliver opportunistic health promotion using opportunities as identified with individual patients.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions.
  • Assess and care for patients presenting with uncomplicated wounds.
  • Perform routine procedures within your scope of competencies such as treating wounds and applying or removing dressings.
  • Taking urine and blood samples, other swabs and specimens.
  • Recognise, assess and refer patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health.
  • Liaise with other health professionals and multidisciplinary teams in the best interest of the patient.
  • Utilise own skills and knowledge to deliver safe, evidence-based practice, adhere to local and National policies and NICE Guidelines.
  • Implement and participate in vaccination and immunisation programmes for adults, such as Flu vaccines, pneumococcal and shingles vaccines to patients accessing the surgery.
  • Undertake community visits for housebound patients as identified individually and within own scope of practice.
  • Restocking and maintaining consulting and clinical rooms if stock supplies are low, as appropriate.

Person Specification:

  • Nursing Qualification.
  • Registered with the NMC.
  • Diploma or above in Diabetes.
  • Non-medical Prescribing.
  • Disclosure and Barring Service Check.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
