Are you a friendly and client focussed Support Worker with a passion for supporting people and keen to make a positive difference? We have a fantastic opportunity for an enthusiastic Support Worker to join our team based in Gillingham, Kent on a permanent basis, working 37.5 hours per week. Shifts are 12.5 hours across a 7 day period. The successful candidate will receive a salary of Β£24,727 per annum as well as other generous benefits.

KEY RESPONSIBILITIES
- Key work and support clients generally within your service
- Perform individual personal care tasks, including washing, showering and toileting, medication administration and promoting independence.
- Regularly fully utilise the Trust Client File with designated clients to ensure that they are supported and empowered to achieve appropriate outcomes.
- Write care plans and ensure these are followed and handed over to all staff, reporting any concerns to the Management Team.
- To maintain a clear and calm manner whilst managing client risk through assessment, planning, client knowledge and de-escalation as required.
- Ensure that all medication within the service is managed in line with Trust policy and procedures.
- Provide support and guidance to other Support Workers to assist them in the safe delivery of the service and to support client engagement.
