The post holder will provide Clinical Care-Coordination for the population of Lancaster Medical Practice and Lancaster Integrated Care Community in line with the Better Care Together Strategy. The post-holder will be pivotal to the ICC and will work alongside the Practice and other providers.
Key Responsibilities
This role requires a flexible approach, willingness to try things out, and an ability to accept a changing remit. The Integrated Care Community is a multidisciplinary team with a geographical footprint based within primary care communities. The aim of this team is to proactively manage clients, to prevent hospital use and allow early hospital discharge. The team model places the client and their experience firmly at the centre of the model and encourages self-support. The ICC wraps around the client and offers a simple interface where the client is able to access the right support, in the right place, at the right time. The post holder will be based at Lancaster Medical Practice but the work involves various locations across the locality and will work both as part of the Lancaster PCN Team and a wider multi‑disciplinary group, playing a lead role in coordinating support and support to all clients within the ICC. They manage a short term caseload of identified clients, to ensure that appropriate support is delivered and that unplanned, changing needs are addressed. The post holder is expected to act as an advocate for clients.
