This is an incredibly exciting time to join our service. We have launched Living Better Lives in Lancashire, our new strengths-based vision for Adult Social Care. Through this, we are embedding innovative practice that will make a real difference to the lives of people in our communities. We are looking for Social Workers to support us to deliver our vision of supporting people to achieve what matters to them, living independently in a place they call home.
Collaboration and co-production are at the heart of what we do. Working alongside partners, community organisations, carers, and most importantly the people we support to shape meaningful and personalised outcomes.
The role: Joining our Intermediate Care Team as a Social Worker, you will support people with rehabilitation, recovery and reablement following illness or hospital admission. Working within CQC-regulated settings, the focus is on maximising independence, supporting timely step-down from hospital and enabling people to return home wherever possible, with safe, person-centred care that promotes recovery and wellbeing.

Social Work applicants must be registered with Social Work England (SWE) to be appointed as a Social Worker or have applied to register with SWE.
