An experienced Social Worker with empathy and the drive to make a difference, finding creative ways to support people to achieve what they want to and live as independently as possible in a place they call their home.
We need a permanent, full time, Social Worker to work across Preston, Chorley & South Ribble as part of our Urgent Care Acute and Prisons Service
Living Better Lives in Lancashire
This is a hugely exciting time for us. We\'ve launched Living Better Lives in Lancashire – our new strength-based vision for social care.
We\'re driving forward innovative practice that will make a difference to our communities and we need new social workers to help us achieve our vision of supporting people to achieve what they want to, living independently in a place they call their home.
The team provides a single point of access to community intermediate care services, including reablement and residential rehab, 7 days per week. The team also supports hospital discharges and works with the wider multi-disciplinary team to prevent hospital admissions and supports people to remain at home. The team also utilises the Discharge to Assess model to ensure that assessments are done away from the hospital environment.
This key role will also involve holistic, face-to-face Care Act Assessments, reviews and support planning, using a strengths-based approach to ensure that people receive the right support at the right time, enabling them to achieve their optimum level of independence and live their life in a way that they choose.
