Salary: £36,363 – £46,142 per annum | Hours: 37 per week | Location: Preston, Lancashire

Responsibilities: You will:
Work with individuals in the community to help them build on strengths, maintain independence and achieve personal goals.
Support people in living independently in a place they call home.
Collaborate with the team to deliver person‐centred care and innovative practice.
Qualifications:
Register with Social Work England (SWE) or be in the process of registering.
Have a valid car for use in connection with duties or be willing to provide driving if unable to drive due to a disability.
Be subject to a Disclosure and Barring Service (DBS) \’disclosure\’ (R of Offenders Act 1974).
