We are recruiting Social Workers with empathy and the drive to make a difference, finding creative ways to support people to achieve what they want to and live as independently as possible in a place they call their home. This role is fixed-term, full-time, until September 2026, working across West Lancashire as part of the Adult Services Mental Health Team.

Key Responsibilities

  • Undertake and review assessments of need, risk and capacity.
  • Sourcing and commissioning care, undertaking duty tasks and supporting discharge planning.
  • Work as part of the wider multi-disciplinary team.
  • Collaborate and coproduce with internal/external professionals, community services, carers and the people we support as part of the Community Mental Health Transformation in Lancashire.
  • Work with adults with a range of mental health difficulties who require statutory interventions.
  • Opportunities for development and progression, including Approved Mental Health Professional Training.
