About the Role
As an Integrated Lead you will provide operational leadership across ICAT services, ensuring effective coordination of care pathways, supporting timely interventions and safe transitions. You will work collaboratively with multidisciplinary teams to deliver integrated, outcome‑focused support and drive service improvements aligned with Adult Social Care priorities and statutory requirements.
Key Responsibilities
- Provide operational leadership across ICAT services.
- Ensure effective coordination of care pathways, supporting timely interventions and safe transitions.
- Work collaboratively with multidisciplinary teams to deliver integrated, outcome‑focused support.
- Drive service improvements aligned with Adult Social Care priorities and statutory requirements.
- Lead and manage staff within the integrated teams, fostering a culture of collaboration and continuous improvement.
- Monitor performance and quality standards, ensuring compliance with policies and best practice.
- Support strategic objectives for short‑term interventions, reducing hospital admissions and promoting independence.
- Engage with stakeholders, including health partners, to strengthen integrated working.

You must be registered (SWE) to be appointed as a Social Worker or have applied to register with the Social Care Council. You will be required to provide a car for use in connection with the duties of this post and must be insured for business use.
