Role

Referral Officer – International Sponsorship – this role is not open to international sponsorship

Responsibilities

  • Provide support to conduct preadmission assessments for people leaving hospital who wish to undertake a period of rehabilitation within one of the Older People Intermediate Care Services.
  • Manage and co-ordinate residential rehabilitation referrals and maintain all aspects of the service's data collection/recording requirements.
  • Liaise directly with individual residential establishments and other health colleagues and partners to streamline the referral process.
  • Work with the team to support service improvement and development within health and social care settings.
  • Support duties within the rota as outlined and ensure coverage in line with service needs.

Rota

  • Hours 09:00 – 17:00
  • Week 1 – Mon, Tues, Weds, Thurs, Sat
  • Week 2 – Sun, Tues, Weds, Thurs, Fri
  • Week 3 – Mon, Tue, Weds, Thurs, Fri
  • Week 4 – Mon, Tue, Weds, Thurs, Fri

Qualifications & experience

  • Experience working within a health and social care setting.
  • Experience with supporting projects and driving quality improvement and development.

Our committed team provides care to more than 1,000 adults across Lancashire, with 16 Care Homes and 13 Day Centres supporting older people, adults living with dementia and their families. We have been recognised for customer service excellence.
