Salary £36,363 - £40,777; Permanent – Social Worker on the Intermediate Care Team delivering Adult Social Care.
What the Role Involves
Working within CQC‑regulated settings, you will support people through rehabilitation, recovery and re‑ablement after illness or hospital admission. The focus is on maximising independence, facilitating a timely step‑down from hospital, and enabling individuals to return home wherever possible with safe, person‑centred care that promotes recovery and wellbeing.
