We are seeking an empathetic and driven Senior Social Worker to join our Urgent Care Acute and Prisons Service in Morecambe Bay on a full time, permanent basis.

We are driving forward innovative practice that will make a difference to our communities and we need new social workers to help us achieve our vision of supporting people to achieve what they want to, living independently in a place they call their home.
Key Responsibilities
- Provide a single point of access to community intermediate care services, including reablement and residential rehab, 7 days a week.
- Support hospital discharges and work with the wider multi-disciplinary team to prevent hospital admissions.
- Utilize the Discharge to Assess model for assessments away from the hospital environment.
- Conduct holistic, face-to-face Care Act Assessments, reviews, and support planning using a strengths-based approach.

- Registration with Social Work England (SWE) or application to register with SWE.
- The post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (as amended in 2013). Successful candidates will need to apply to the Disclosure and Barring Service (DBS) for a 'disclosure'.
