We’re driving forward innovative practice that will make a difference to our communities and we need social workers to help us achieve our vision of supporting people to live independently in a place they call home.

Role Responsibilities

  • Undertake strength‑based, outcome‑focused assessments and support planning to ensure residents receive the right amount of support.
  • Support people to access local services, maintain independence, promote health and wellbeing, and live independently.
  • Work alongside other agencies, professionals, and informal carers to provide a holistic service.
  • Receive training and progression opportunities in a supportive environment.
