Referral Officer – International Sponsorship: this role is not open to international sponsorship.

Lancashire County Council's Older People Care Services invites applications for the position of Referral Officer. This is a permanent, full-time role (37 hours per week) with a salary of £28,142 - £32,061 per annum. The post holder will work from County Hall in Preston with potential to work from home or other locations.

Responsibilities

  • Provide support to conduct preadmission assessments for people leaving hospital who wish to undertake a period of rehabilitation within one of the Older People Intermediate Care Services.
  • Manage and co-ordinate residential rehabilitation referrals and maintain all aspects of the service's data collection and recording requirements.
  • Liaise directly with individual residential establishments and other health colleagues and partners to streamline the referral process.
  • Work to support projects and contribute to quality improvement and development within the service.

Rota and working hours

Hours: 09:00 – 17:00

Week 1 – Mon, Tue, Wed, Thu, Sat
Week 2 – Sun, Tue, Wed, Thu, Fri
Week 3 – Mon, Tue, Wed, Thu, Fri
Week 4 – Mon, Tue, Wed, Thu, Fri

Qualifications and experience

  • Experience working within a health and social care setting.
  • Experience with supporting projects and driving quality improvement and development.
