Overview

Are you an highly experienced, enthusiastic, organised and motivated speech & language therapist with a specialist interest in stroke and neuro?

Background

We looking for an experienced speech & language therapist with advanced clinical skills to be part of the established Community Neurological Rehabilitation Team (CNRT) in Central Lancashire. The CNRT provides ongoing rehab to patients within the Chorley, South Ribble and Greater Preston locality with predominantly Stroke and ABI diagnosis.

Responsibilities

You will have a commitment to working within an interdisciplinary environment, who has experience of working with patients with stroke and neurological conditions.

You will be offered regular supervision by a highly experienced therapist to support you to develop and expand on your existing skills and knowledge.

You will be responsible for the assessment, treatment planning and management of rehabilitation programmes for a wide variety of clinical presentations and will be responsible for delegation to generic rehabilitation assistants and support them in the delivery of this rehabilitation.

You will manage your caseload and diary. You will be offered regular in-service training as well as support for internal and external development within the field of Neurological Rehabilitation and leadership.

You will develop close partnership working with inpatient colleagues and other professionals.

Due to the nature of the role, it is essential that you can travel independently across the locality.

To be responsible for the assessment, treatment, planning and delivery of evidence–based rehabilitation interventions.

To work as part of the rehabilitation and or stroke/neurological service line and be responsible for assessment and management of a clinical caseload of patients with complex needs.

To undertake comprehensive assessment of patients with complex presentations, using investigative and analytical skills, to formulate an appropriate hypothesis and to determine a diagnosis and appropriate management plan.

To establish individualised management and treatment plans, using clinical reasoning and utilising a wide range of evidence-based treatment skills and options to formulate highly specialised interventions and programmes of care, which may include working with staff across organisational boundaries.

To promote health and reduce inequalities for all patients accessing the service, so that they may be able to maximise their potential in terms of health education and social well-being.

Contribute to clinical leadership and supervision facilitating a culture of learning and reflective practice within podiatry.

Document and evidence patient outcomes.

To lead/contribute to training and development programmes as appropriate to the knowledge and skills of the individual, delivering to other professionals both within and outside the NHS and service users.
