An exciting opportunity to join Lancashire South Cumbria Foundation Trust’s (LSCFT) Central and West Network as a Senior Occupational Therapist/Physiotherapist. The role sits within acute therapy delivery within Care Connexions as part of the Urgent Community Response, Virtual Ward and Home First teams, focusing on admission avoidance work in the community and early supported discharge from the acute hospital by providing a therapy assessment of patients' function and care needs in their home environment following the principles of the discharge to assess model.

The teams work collaboratively with other social care providers to provide an integrated health and social care team working together to ensure the most appropriate intermediate care services are commissioned based on the patient’s needs. You will improve patient and family experience during the discharge process, promote continuity of care from the acute into the community by supporting an integrated approach, and enable patients to recover within their home environment after assessment of needs.

Responsibilities
  • Provide therapy assessment of patients' function and care needs in their home environment.
  • Work within the Urgent Community Response, Virtual Ward and Home First teams.
  • Promote continuity of care from acute to community and support an integrated approach.
  • Support patients to recover in their home environment.
  • Undertake clinical triage supporting timely discharges from acute and admission avoidance triage.
  • Collaborate with other health and social care providers to commission appropriate intermediate care services.

Working Hours

Part‑time hours may be considered. Hours may be required to be worked over 7 days between 08:00 and 20:00, depending on service need. Flexible working patterns such as term time, part time, compressed hours and flexi‑time will be considered.
