We are excited to be extending our team at the HBPOS Lancaster and are looking to appoint to B3 Health Care Support Worker post for the HBPOS in the Access and Urgent Pathway, supporting patients in Health Based Place of Safety.
To work as a member of the Health Based Place of Safety team providing a high standard of intervention in line with service objectives over a 7 days week 24 hour rota supporting the urgent care pathway.
To assist the Health Based Place of Safety practitioners to assess, plan, implement and evaluate individual care plans. To work with service users who are detained under a Section 136 or 135 of the MHA, who are in crisis and have been brought into the Health Based Place of Safety by the police and are awaiting an assessment.
To monitor and assess risks and report back to the Mental Health Practitioner as required. To assist Mental Health Practitioners and the wider MDT during the assessment and following the assessment to ensure a safe discharge plan is implemented.
Ensure the smooth transition for service users between specialist and mainstream services and other teams.
- You will need to have experience working with patients and a Care Certificate or equivalent level learning.
- You will be able to work with a wide range of people.
- The teams work 24/7 so you will need to work shift patterns.
- We work with people and their families and support networks to support people at home and reduce hospital admissions.
- You will be part of a team of experienced practitioners and you will be supported to develop your role and knowledge.
- You will receive training and regular supervision.
- You will be part of the development of the team.
