To support the provision of a community service to individuals with a learning disability in a defined area of Lancashire, ensuring health care needs are met and contractual agreements fulfilled by a registered learning disability nurse.
Key Responsibilities
- Support the delivery of community services for people with learning disabilities.
- Manage a defined caseload and supervise junior staff.
- Implement evidence‑based and innovative practice, including positive behavioural support.
- Work with individuals who have complex health needs, challenging behaviour and/or Autism.
- Collaborate with other agencies within the Adult Community Learning Disability Service.
Qualifications & Skills
- Registered learning disability nurse with current nursing registration.
- Knowledge of health policy and legislation applicable to learning disability and/or Autism.
- Experience supporting people with complex health needs, challenging behaviour and autism.
- Demonstrated evidence‑based and innovative practice.
- Excellent communication, adaptability and self‑motivation.
- Capability to supervise junior staff.
