Lancashire and South Cumbria NHS Foundation Trust is seeking a Specialist Dietitian to join the Community Dietetic Team. The post is based at Moor Park Health and Leisure Centre, Blackpool, with flexibility to travel around the Fylde Coast.
You will assess and implement care plans for patients at home, in care homes and community settings, train health care professionals, and contribute to IT systems that support patient care within a multidisciplinary team.
