We are looking for an experienced qualified paediatric nurse to join our welcoming, dynamic, and supportive Children’s Continuing Care and Assessment Team enabling children with complex physical health needs to live well in the community.

Role Responsibilities

  • Support children with complex physical health needs in their homes by completing Health Needs Assessments under Continuing Care criteria.
  • Support ongoing medical device and consumable supply, training, advocating, and supporting the child or young person and their family.
  • Work closely with the wider children’s nursing teams in LSCFT and with the wider MDT supporting the child or young person’s individual needs.
  • Undertake assessments of children and young people to establish individualised management and treatment plans and determine appropriate interventions and goals.
  • Have the appropriate clinical skills and experience to support training at home, in schools and in nurseries if a need is identified.

Candidate Requirements

  • NMC registered with post‑qualification children’s nursing experience including working with children with complex health needs in a multi‑agency context.
  • Possess excellent clinical and communication skills and have knowledge and experience of children’s complex needs nursing and the technology and equipment used by these children.
  • Great organisational skills and ability to work independently accessing supervision as required.
  • Highly motivated and dedicated to evidence‑based practice.

Working Location and Travel

The team covers East Lancashire, Blackburn with Darwen, Preston and West Lancashire. This post will be based in either the Burnley or Preston office but travel across this area will be essential to this post on a regular basis.
