Don't miss out on this exciting opportunity to join the expanding joint working across Home First and Urgent Community Response Services to facilitate complex hospital discharges and to support and keep patients safe in their own home. We are a team of Occupational Therapists, Physiotherapists and advanced clinical practitioners working closely with the multidisciplinary team to prevent further deterioration in patients' physical functioning and wellbeing. We cover the areas of Chorley, Preston and South Ribble.

Your role will be to support the team including undertaking non‑complex assessments and carrying out treatment plans under the direction of the therapists.

Key Responsibilities
  • Work with patients in their own homes.
  • Operate 365 days a year, 8.00am – 8.00pm.
  • Undertake non‑complex assessments and carry out treatment plans under therapist direction.
  • Provide support to the multidisciplinary team.
  • Work autonomously as well as part of a team.
  • Attend regular supervision and professional development opportunities.
Qualifications & Experience
  • Assistant Practitioner qualification or equivalent.
  • NVQ Level 3 or equivalent.
  • Evidence of continued professional development.
  • Knowledge of long term conditions.
  • Knowledge of mental illness and disability.
  • Knowledge of the cultures and religions of the area communities and the ability to work across those cultures.
  • Experience of working in the community.
  • Demonstrates the ability to work with minimum supervision.
  • Is able to organise own time and workload.
  • Proven problem‑solving and clinical reasoning skills.
  • Able to adapt communication style to suit different people, including use of verbal and non‑verbal communication.
  • Physically capable of carrying out the duties required, including manual handling of equipment or household items.
