As the Practice Lead Nurse, you will oversee the HCA and Care Coordinator team, providing clinical leadership, guidance, and support. You will be the Infection Prevention and Control (IPC) Lead for the main operational site at Akerman Health Centre, the branch site at Hurley Clinic, and any future practice locations. You will be responsible for delivering high-quality nursing care, managing minor illnesses, and providing health promotion and disease prevention advice. You will contribute to the effective delivery of Quality and Outcomes Framework (QOF) targets, actively support the seasonal flu vaccination campaign, NHS Health Checks, Learning Disabilities, Diabetes, etc.

Job responsibilities

  • Clinical Responsibilities
    • Diagnose and treat patients presenting with minor illness within scope of competence and NMC Code of Practice.
    • Deliver evidence-based care following NICE and NSF guidelines.
    • Support and promote patient self-care and adherence to treatment.
    • Provide chronic disease management (Diabetes, COPD, Asthma, Hypertension).
    • Perform foot checks, blood pressure checks, urinalysis, wound care, compression bandaging, and immunisations (including baby immunisations).
    • Carry out cervical screening, contraception reviews, health checks, and dietary advice.
    • Undertake phlebotomy, injections (e.g. B12, Depo-Provera, Clopixol, Prostap, Zoladex), and assist in minor procedures.
    • Manage vaccine stock, storage, and ordering processes.
    • Oversee and coordinate the HCA and Care Coordinator teams to ensure safe and effective patient care delivery.
    • Delegate duties appropriately within the nursing team.
    • Provide supervision, mentorship, and support to HCAs and new clinical staff.
  • Quality and Outcomes Framework (QOF) and Public Health Programmes
    • Support the delivery and achievement of QOF targets, including data collection, clinical recall, and evidence-based care.
    • Participate in annual QOF reviews and contribute to improvements in chronic disease management.
    • Lead and support the seasonal flu campaign, including patient recalls, vaccine administration, data recording, and staff coordination.
    • Assist in other immunisation and vaccination campaigns as required (e.g. COVID-19, shingles, pneumococcal).
  • Infection Prevention and Control (IPC)
    • Act as IPC Lead across all Lambeth Walk Group Practice sites.
    • Lead on all infection control audits, staff training, and implementation of safe clinical practices.
    • Ensure the practice complies with CQC standards and NHS Infection Control requirements.
    • Maintain a clean, safe, and compliant clinical environment.
    • Collaborate with the Practice Manager on regular IPC audits (e.g., hand hygiene, sharps, waste management).
  • Administrative and Professional Duties
    • Maintain accurate, contemporaneous clinical records.
    • Participate in audit, quality improvement, and service development activities.
    • Support practice meetings and contribute to multidisciplinary discussions.
    • Manage clinical stock, emergency drugs, vaccines, and equipment monitoring.
    • Handle patient recalls for smears and immunisations.
    • Handle patient recalls for immunisations due and overdue.
    • Coding blood spots.
    • Participate in appraisals, training, and continuing professional development.
  • Health & Safety and Infection Control
    • Promote and monitor safe working environments across all clinical areas.
    • Ensure correct use and management of PPE and sharps.
    • Implement and uphold infection control policies and decontamination procedures.
    • Report and manage any clinical risks or incidents appropriately.
  • Confidentiality, Equality, and Diversity
    • Maintain strict confidentiality in line with GDPR and practice policy.
    • Treat all patients and colleagues with respect and professionalism, valuing equality and diversity.
  • Training and Development
    • Participate in annual appraisal and maintain a Personal Learning Plan.
    • Attend mandatory and relevant clinical training to maintain NMC registration.
    • Support the induction and training of new clinical team members.

Person Specification

  • Experience
    • Registered Nurse with experience of working as a qualified Nurse.
    • Experience of prioritising needs and making informed decisions in care delivery.
    • Experience of developing services based on client needs.
    • Knowledge of current primary care organisational policy and national legislation.
