We areseeking a dedicated and professional Clinical Pharmacist to join our multidisciplinaryteam.
This is aunique opportunity to contribute to a forward-thinking organisation within thePrimary Care Network (PCN) framework. The PCN has already established excellentcollaboration in delivering pharmacy provision to the many aspects of the DESwithin its locality. The post holder will help underpin and grow the team tomove to working in line with the ever-evolving requirements of PCNs.
Our currentteam is complemented by clinical pharmacists and pharmacy technicians workingacross all sites. This role is a replacement within the team to continue togrow on the support and accessibility of our PCN and its patient population.

Main duties of the job
Key Responsibilities:
- Clinical Medicines Management
Provide expertise in clinical medicines management, including medication reviews and ensuring safe prescribing practices, especially during care transitions.
- Medication Reviews
Conduct remote or face-to-face structured medication reviews for patients, particularly those with polypharmacy, long-term conditions, or in care homes
- Support for Patients
Offer patient-facing support, addressing medication queries and optimizing medication use to improve adherence and outcomes
Reconcile and manage medicines post-hospital discharge, ensuring continuity of care and addressing unexplained changes
- Quality Improvement
Lead clinical audits and contribute to quality improvement initiatives, including adherence to local and national prescribing guidelines.
- Service Development
Provide pharmaceutical advice for creating and implementing new services with medicinal components
Review and improve the safety of medicines associated with unplanned hospital admissions and high-risk groups
- Education and Guidance
Provide medication-related information to practice staff, healthcare teams, and patients, suggesting solutions and monitoring outcomes.
- Policy Implementation
Monitor compliance with local and national guidelines, assisting with practice formulary updates and adherence to NICE guidance.

South Peterborough PCN is a large, forward-thinking PrimaryCare Network compromising of 4 practices Yaxley, New Queen Street &Stanground, Old Fletton and Wansford.
This role is based across the PCN.
Job responsibilities
The post holder will work within their clinical competenceas part of a multi-disciplinary team to provide expertise in clinical medicinesmanagement. This involves providing face to face structured medication reviews,managing long term conditions, ensuring safe prescribing at the transfer ofcare, implementing systems for safer prescribing, repeat prescriptionreauthorisation and dealing with acute prescription requests.
Person Specification
Qualifications
- Registration as a Pharmacist with the General Pharmaceutical Council
- Evidence of CPD
- Completion of CPPE pathway, or working towards completion
- Independent prescriber or working towards
- Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
Experience
- Minimum of 2 years post-qualification experience.
- Experience of 1 year working in general practice.
- Experience of using SystmOne.
- Experience of working across multiple sectors of Pharmacy (Community Pharmacy, Hospital Pharmacy, CCGs, Industry).
