THE HEREWARD PRACTICE, BOURNE is looking to recruit a prescribing nurse practitioner to work 37.5 hours a week to help care for our patients during a period of maternity leave for up to 12 months.

Main duties of the job

The ideal candidate will demonstrate critical thinking in the clinical decision-making process. They will work autonomously but collaboratively with multi-professional teams to meet the needs of patients, supporting the delivery of policy and procedures.

In summary, the post holder is an experienced prescribing practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.

The practice looks after 12,500 patients and our nurse practitioner team is a small supportive team responsible for providing complete episodes of care for patients with a wide variety and range of presenting problems and health care needs. This encompasses the provision of evidence based, high-quality care for patients whose issues fall within urgent/acute/chronic episodes and including health promotion and public health elements.

Job responsibilities

Supporting the overarching Lakeside Healthcare Group strategy and service delivery, the post holder will provide complete episodes of care for patients with a wide variety and range of presenting problems and health care needs. This encompasses the provision of evidence based, high-quality care for patients whose issues fall within urgent/acute episodes, long-term/chronic conditions and including health promotion and public health elements.

Working within the NMC Code of Conduct and utilising primary care skills and experience, the post holder will provide advanced care interventions. Using evidence-based knowledge, the post holder will deliver a high standard of care, making professionally autonomous and accountable decisions. The post holder will work within their professional boundaries to demonstrate safe clinical decision-making and cost-effective, expert care for patients within the practice.

The post holder will support service development, the safe delivery of policy and procedures and demonstrate effective nurse/AHP advocacy and leadership.

Person Specification

Experience

  • Experience of working in an independent and autonomous role, diagnosing and treating patients
  • Experience of providing care and management of patients with complex needs
  • Evidence of wide knowledge base and competency in clinical practice
  • Experience in Primary Care

Qualifications

  • Registered Nurse NMC - 1st level Registration
  • Degree Level qualification in Advanced Nursing Practice, Community Nursing or Specialist Nursing or Diploma in Emergency Care Practitioners course, Diploma of higher education (DipHE) or degree in paramedic science or paramedic practice
  • Accredited physical assessment course
  • Evidence of continued professional development
  • Accredited training in chronic disease management

Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

Salary

£44,993 to £53,948 a year. Salary will be dependent on experience.
