The role
You’ll visit service users in their own homes across Yarm and the surrounding area, delivering care that is person-centred, dignified and genuinely tailored to each individual. Visits typically include:
- Personal care - washing, dressing, continence support and oral care, always with dignity and respect
- Medication administration and prompting, in line with each service user's care plan
- Companionship and wellbeing visits - conversation, activities and simply being a reassuring presence
- Dementia and complex care, supporting service users and their families with patience and skill
- Reablement and supported independence - helping people regain confidence after illness or hospital discharge
- Meal preparation, light domestic support and accompanying service users to appointments
- Accurate, timely care notes using our digital care system
