To work within the community nursing service to support patients with diabetes, as part of a team collaboration with Wirral University Teaching Hospital's diabetes service. The post-holder will have managerial and clinical supervision from WCHC, with access to specialist clinical supervision and professional development from the Wirral University Teaching Hospital team.
This work will include:
- Supporting discharges from hospital for patients with complex insulin regimens as part of a multidisciplinary team, ensuring safe, timely and well-coordinated discharges.
- Developing and delivering training and advice in diabetes and diabetes management to staff in and beyond district nursing, including titration of patients on insulin.
- Providing direct specialist diabetes nursing assessment and support (including for self-management) to their own caseload of patients, typically housebound, and including those on palliative care pathways, ensuring their diabetes management needs are met.
- Conducting joint home visits and providing specialist clinical patient assessments to support frontline clinical staff in delivering high quality, safe, integrated community care.
- Contributing to the development of best practice and facilitating shared learning to develop services and initiatives that meet local needs in relation to Wirral Community Health and Care NHS Foundation Trust aims and objectives.
- Supporting the development of a self-care pathway for patients with diabetes on the community nursing caseload within the Nursing Directorate.
