We welcome applications from all backgrounds and particularly from groups currently under-represented, including people with disabilities, those from minority ethnic communities or from socio-economically disadvantaged backgroundsThe successful candidate will be based within the Integrated Discharge Hub – Hospital Social Work Team delivering a service to patients in St John’s Hospital to help facilitate timely discharge from hospital and support people to remain in the community, where they are medically well but need sort term support due to a change of circumstancesThe Integrated Discharge hub is an inter – agency multi – disciplinary team made up of hospital social workers, community care co-ordinators, reablement staff, discharge facilitators, AHP staff as well as representatives from different services.This is an exciting opportunity for the right person to work with a range of professionals and people to support the principles of Home First.The worker must have the ability to work as part of a team in a pro-active manner identifying opportunities for Home First whilst ensuring the patient has the appropriate level of support to discharge from hospital.Tasks will be carried out in partnership with service users, family members, colleagues within Social Policy and other professionals and agencies as necessary to meet need. The worker will be required to have a good understanding of Self-Directed Support and the importance of completing outcome focused assessments.The worker will be required to undertake essential training including in adult support and protection training.The worker must have the ability to travel throughout West Lothian.Before confirming appointment, you will be required to obtain PVG Scheme Membership through Disclosure Scotland.The skills and knowledge candidate must have: -* SVQ 3 or HNC in Social Care or equivalent or relevant experience in a social or health care setting is essential.* It is desirable for the candidate to have relevant assessment and care management experience in a social work practice team.This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007.
