We are a friendly GP Practice seeking a GP Assistant for the role of Care Navigator. The Care Navigator will work closely with the administrative team to support daily triage of incoming patient calls and requests, with potential for later involvement in chronic disease clinics. Support and training will be available via the GP partners and senior administrative team.
About Us: KS Medical Centre is a SystmOne practice within the North Southall Primary Care Network in Ealing Borough, with ~5,000 patient population. We have been involved in Imperial College undergraduate medical training and recently received approval for GP registrar training. Our clinical team of doctors, practice nurses and healthcare assistants is supported by local physiotherapists, social prescribers and visiting paramedics for home visits. We also have our own pharmacist service for virtual medication review and prescription authorisation. Our patient population is ethnically diverse and presents a range of chronic disease and acute presentation challenges.
Responsibilities:
- Collaborate with the administrative team to triage patient calls and requests.
- Serve as the first point of contact for patient triage.
- Follow practice protocols and guidelines developed by the senior clinical team.
- Physical presence in the practice is required; own transport not essential.
Qualifications & Experience:
- Fluent in English.
- Computer literate.
- Basic medical understanding.
- GCSE Grade A‑C in English and Maths.
- Qualified to NVQ level 3.
