  • Main Roles & Responsibilities
  • You will be helping people live life to the full; developing positive relationships with team members and those you support is a key part of your responsibilities.
  • Everyone is an individual and the support you provide must align to this need.

One day you may be helping a person with their finances and to shop, the next you may be supporting the person to a cinema, day trip or even a holiday.

  • You will be working with service users on a needs led basis and delivering specific support needs alongside the person’s care plan.

In general terms this will be supporting people with life skills, helping people develop and attain a level of independence, and supporting people to be active citizens in their local community.

  • You will be responsible for documenting any observations alongside the person’s care plan to form part of the multidisciplinary team.

You will be attending meetings to ensure the best possible outcomes can be achieved in respect of health, social need and person centred aspirations.

  • Must have
  • Good communications skills both written and verbal
  • Good level of English and Maths
  • Ability to build good rapport with service users, outside agencies and staff.
  • Patience, Attention to Details and Highly Organised
  • Ability to work as part of a team and use own initiative

Qualifications Required

  • Minimum level equivalent to Grade C English & Maths
