About the Role
  • Work with adults with a wide range of needs, including mental health, physical disabilities, older adults, and carers.
  • Complete strengths-based assessments and develop person‑centred support plans.
  • Undertake safeguarding enquiries, applying professional curiosity and sound judgement.
  • Work collaboratively with health partners, voluntary agencies, carers and families.
  • Promote independence, wellbeing and choice, in line with the Care Act 2014.
  • Contribute to a positive team environment where learning and reflective practice are encouraged.

Qualifications and Experience
  • Qualified Social Worker registered with Social Work England.
  • Completed your ASYE and are ready for more complex, varied casework.
  • Confident in applying the Care Act 2014 and related legislation.
  • Strong assessment, safeguarding and risk management skills.
  • Values partnership working and person‑centred approaches.
  • Committed to continuous professional development and reflective practice.
