Responsibilities
- Work with adults with a wide range of needs, including mental health, physical disabilities, older adults, and carers.
- Complete strengths-based assessments and develop person centred support plans.
- Undertake safeguarding enquiries, applying professional curiosity and sound judgement.
- Work collaboratively with health partners, voluntary agencies, carers and families.
- Promote independence, wellbeing and choice, in line with the Care Act 2014.
- Contribute to a positive team environment where learning and reflective practice are encouraged.

Qualifications
- Are a qualified Social Worker registered with Social Work England.
- Have completed your ASYE and are ready for more complex, varied casework.
- Are confident in applying the Care Act 2014 and related legislation.
- Can bring strong assessment, safeguarding and risk management skills.
- Value partnership working and person-centred approaches.
- Are committed to continuous professional development and reflective practice.
