If yes, this could be the opportunity for you! We are seeking to appoint a dynamic and motivated individual to the role of Transitions Keyworker. This is an exciting opportunity to join an expanding area of the curriculum, which boasts continued learner success and achievement.

Responsibilities

  • The successful candidate must ensure that students with an EHCP make a successful transition from schools and other post‑16 provision into College.
  • This involves working closely with other agencies, institutions and parents to design a transition plan for Supported Learning students and ensuring they have a fully supported transition into College.

Qualifications and Experience

  • SEND qualification
  • Maths and English to Level 2 or a willingness to work towards.
  • Experience of working with young people in an advisory or support role
  • Full understanding of the SEND Code of Practice and the expectations and duties towards young people regarding transition
  • An understanding of the special educational needs, disabilities and other barriers that young people may present within school.
  • Ability to profile and assess needs, to devise a Transition Plan and contribute ideas for appropriate support from college.
  • Ability to build rapport with young people and offer unbiased advice to support their next steps in education.
  • Knowledge and understanding of EHCPs and the role of colleges.
  • Excellent written skills including excellent spelling and grammar
