Permanent role in the Urgent Care Response Service (Joint Emergency Team, JET) based in the Royal Borough of Greenwich. The JET team delivers an agile, robust hospital admission avoidance service, focusing on health and social care crisis interventions in the community. The team consists of professionals from Oxleas Community Health Services and the Royal Borough of Greenwich Adult Social Care Service, including GPs, nurses, occupational therapists, physiotherapists, social workers, pharmacists, and a handyman. Working within this multidisciplinary team offers a comprehensive wrap‑around service and the flexibility to collaborate across a range of expertise.
Key Responsibilities
- Maintaining up‑to‑date working knowledge of adult social care legislation and implementing best‑practice principles.
- Supervising staff and managing day‑to‑day operational workflow.
- Assuring quality of Care Act Assessments and acting as Adult Safeguarding Manager.
- Carring a small caseload, often co‑working on complex cases and providing advice, support and guidance to colleagues.
- Managing out‑of‑hours duties, including leading social work staff on one in four weekends, and covering late shifts (12:30 pm to 8:30 pm) as required.
- Participating in the Out‑of‑Hours on‑call social worker service (8:30 pm to 8:00 am, Friday to Friday, once a month).
- Contributing to a culture of strength‑based practice and supporting continuous professional development for staff.
Qualifications & Experience
- Registered with Social Work England.
- Experience in supervising and line‑management of social work staff.
- Strong knowledge and application of adult social care legislation and safeguarding standards.
- Ability to work independently within a complex community care environment.
- Excellent communication and teamwork skills.
Working Hours & Schedule
The role includes an out‑of‑hours component: one weekend in four to lead discharge and referral management, plus one or two late shifts per week (12:30 pm to 8:30 pm). Additional on‑call service runs from 8:30 pm to 8:00 am, Friday to Friday, once a month.
