The Transforming Care Programme is now part of a wider approach to Health Equalities for Autistic Children, Young People and Adults and those with learning disabilities. The team works on behalf of the Essex ICB and three local authorities across Southend, Essex and Thurrock (SET) to transform the health and social care system, delivering improved health outcomes for people with learning disabilities and/or autism (LD/A). The Case Manager role is essential to this transformation. It sits across strategic and operational elements of programme delivery, centred on the NHS Long Term Plan for those with the greatest vulnerabilities at risk of institutionalised care. The role works within a hosted team, keeping people safe in the community, preventing unnecessary admissions to mental health beds and facilitating timely discharges.

The Case Manager has specific responsibility for enabling preventative and integrated approaches for autistic individuals and/or those with learning disabilities who display challenging behaviours, including those with a mental health condition. The role ensures community services are coordinated to enable individuals to remain in the community where possible and to avoid institutionalised care, while ensuring well-coordinated and timely discharges from inpatient services.

Key responsibilities include:

  • Providing specialist advice and professional expertise for health professionals and social workers;
  • Demonstrating expert and effective practice in complex situations and working with colleagues to mitigate high risk;
  • Making independent decisions and working across health, social care, education and other agencies to ensure appropriate responses;
  • Troubleshooting barriers and resolving issues to facilitate service delivery;
  • Coordinating community services to keep people safe and reduce unnecessary admissions;
  • Facilitating and sometimes chairing Care and Treatment Reviews for this cohort.

Qualifications

  • Educated to degree level (e.g., Diploma or Degree in Social Work or Nursing) or equivalent.
  • Evidence of continuing professional development and subject matter expertise in a relevant professional area; membership of a relevant Health or Social Care professional body.
  • Significant experience working with people with learning disabilities and/or autism and their families, including understanding of the functioning of Health and Social Care systems and applicable legal/statutory frameworks.
  • Comprehensive and up-to-date understanding of all relevant legislation, including the Mental Health Act.
  • Experience chairing meetings involving clients, family members and/or carers and other multi-agency professionals; making decisions and recommendations on difficult and contentious issues with multiple courses of action.
  • Ability to challenge and negotiate with senior stakeholders on difficult and controversial issues to ensure services and interventions are delivered in line with national policy for people with learning disabilities and/or autism.
  • Excellent communication and interpersonal skills; ability to build and maintain relationships with Health and Social Care professionals, individuals with learning disabilities and families, and providers to achieve positive outcomes.
  • Experience of identifying and implementing national policy with regard to learning disability and autism; researching best practice and implementing it locally.

This role involves travelling around Essex and beyond. A driving licence and car – or the ability to meet the mobility requirements of the role through other means – is essential.
