You have an opportunity to be part of this in the role of STEPS Community Reablement Officer. Could you make a difference to people's lives and work in social care? STEPS team (short term enablement programme) need Community Reablement Officers that can demonstrate the 6 C’s: Care, Compassion, Competence, Communication, Courage, Commitment.
You will support people in their own home for up to 6 weeks following a hospital stay or to prevent a hospital admission. You will assist and support people to live how they choose in a safe environment while promoting independence with practical support as agreed in their support plan, this is called reablement.
Responsibilities will include:
- Providing personal care
- Assisting people with mobility transfers following a movement & handling plan
- Providing support with medication and meals
- Understanding safeguarding and risk assessments
STEPS are regulated by the Care Quality Commission; you will understand how ways of working in the team ensure we maintain the highest standard of care and excellence.
You will need a full Driving Licence and own transport to travel throughout the borough. We value a range of experiences and skills. If you have transferable skills from other roles or voluntary work, we encourage you to apply.
