Organisation Details Organisation: Kirklees Directorate: Adults & Health Service Area: Adults Social Care Operation Hours: 37 Number of Jobs: 2 Location(s): Across Kirklees Position type: Permanent Grade: 9-11 Role Overview This role offers an exciting opportunity to join the Supported Accommodation Assessment Team (SAAT), working with adults across North and South Kirklees care homes, supported living accommodation, and extra care housing. SAAT is a new, substantive team within Kirklees Adult Social Care, developed from an established care home team. You will be joining us at a crucial and exciting time. Key Responsibilities Work primarily with adults in 24-hour care settings, supported living, and extra care housing. Hold a caseload, complete holistic assessments and reviews of needs under the Care Act. Participate in team rotas for duty and safeguarding. Engage with individuals, their carers, and relatives, adopting a strength-based, person-centred approach. Work with adults with diverse social care needs, including older people, physical disabilities, learning disabilities, and mental health. Build professional relationships with care staff and health and social care professionals. Participate in Continuing Health Care meetings.
