An experienced Social Worker will be part of the newly established Accommodation Hub within Adult Social Care, working with adults in care homes and extra care housing across Kirklees. The role requires a holistic, person‑centred approach to support independence and choice while ensuring safety.
Responsibilities
- Work with adults living in care home settings and extra care housing.
- Hold a diverse caseload and carry out holistic assessments and reviews under the Care Act 2014.
- Participate in the team duty rota, responding to urgent needs and supporting colleagues.
- Champion a strength‑based, person‑centred approach, ensuring the voice of the individual, their carers, and representatives is central to all decisions.
- Support adults with a range of needs, including older people and individuals with physical disabilities, learning disabilities, and mental health needs.
- Take a proactive role in identifying and managing safeguarding concerns, working in partnership with others to keep people safe while promoting independence and choice.
