About The Specialist Adult Pathway Team (SAPT) The Specialist Adult Pathway Team (SAPT) support young people, helping them and their families prepare confidently for adulthood. We work closely with Children with Disabilities (CWD) and wider Children’s Social Care teams to ensure a smooth, well‑planned transition into adult services.
What You’ll Do As a SAPT Social Worker, you will build relationships with young people and families; complete holistic, strengths‑based assessments; create personalised support plans; prepare young people for adulthood; and coordinate support with health, education, housing and community partners to ensure seamless service delivery.
Who We’re Looking For We are seeking dedicated, person‑centred Social Workers with experience supporting children or adults with disabilities, strong multi‑disciplinary working skills, and knowledge of the Care Act, Mental Capacity Act, DoLS, SEND legislation and personalised approaches.
