The Adult Social Care Teams support adults to maintain their health and wellbeing, maximizing independence, choice and control. The Hospital Social Care team is located in Dewsbury District Hospital and supports adults with complex health and social care needs. The team provides assessments, support packages and early reviews to Kirklees residents that have been admitted to hospital to facilitate timely discharge.
As a Team Manager, you will lead and manage a team of staff including qualified Social Workers. Hospital discharge is a fast‑paced environment, and the team works to clear‑defined targets and deadlines. You will play a key role in ensuring the timely completion of assessments and effective discharge planning. You will develop positive, supportive relationships with partner agencies, including the acute hospital trust.
Responsibilities
- Monitor the team’s budget and ensure the council gets best value for care packages and placements.
- Support the team in adopting enabling approaches and appropriate levels of intervention to promote independence and positive risk‑taking.
- Provide regular supervision and ongoing induction, including an annual appraisal.
- Participate in the manager’s on‑call rota (approximately one Saturday every nine weeks).
- Facilitate change management, encouraging positive adoption of new practices.
- Work with two Deputy Team Managers who provide frontline supervision.
Qualifications
- Recognised social work qualification and registration with Social Work England.
- Extensive experience as a social worker in an adult social care setting.
- Experience at deputy or team manager level is ideal.
