We are a very busy Town Centre Practice covering 2 sites and growingrapidly! We are looking to recruit highlymotivated, enthusiastic and versatile staff for our very busy practice. The Care Navigator will work in a reception office navigating our patients via telephone or face to face. Please note this is a very busy practice and we require someone to work hard to meet the requirements of the patient and team.

We are looking for applicants to sell themselves in this application. If successfully shortlisted we would expect applicants to have done some research in to the practice and role. We hope to recruit someone who can grow in to the role and become a long term dedicated member of the team.

Main duties of the job

The post requires you to be flexible, responding to varying demands and changing circumstances. Communication, team work, motivated hard work ethic and a sense of humour are essential qualities to succeed in this role! Previous experience in a healthcare reception office is desirable, however full training will be given. Please note this is a reception office based role. Flexibility to cover additional hours for holidays and sickness are essential.

Job Role One Hours are Monday 8am to 6.30pm, Tuesday 1pm to 6.30pm, Wednesday 8am to 1pm, Saturday 9am to 12.00 (23.5 hours per week).

Job Role Two hours are Wednesday 1pm to 6.30pm, Thursday 8am to 6.30pm, Friday 8am to 4.30pm and Saturday 9am to 12.00 (26.5 hours per week).

We are a training practice, have over 15825 patients and our staff will work across 2 sites, Kingthorne and Edenthorpe branch site. We work together as a team to get the job done and would expect you to do the same. This is a very busy and varied role. The patients are at the forefront of everything we do.

Job responsibilities

JOB TITLE: CareNavigator

DEPARTMENT: Reception

Reports to: Workforce Administrator/Management Team

Job Summary:

Receive, assist and direct patients inaccessing the appropriate service or healthcare professional in a courteous,efficient and effective way.

Provide general assistance to the Practiceteam and project a positive and friendly image to patients and other visitors,either in person or via the telephone

Job Responsibilities:

Administration

  • Tohave a thorough knowledge of all Practice procedures
  • Towork in accordance of written protocols
  • Scanrelevant documents in to patient records

Reception

  • Receivingpatients consulting with members of Practice team, directing to selfarrival where appropriate.
  • Promoteservices to patients ie online registration, patient partner, texting, Accurx.

Appointments

  • Processappointment requests for appointments from patients by telephone, online and inperson.
  • Dealwith visits requests

Computer

  • Registrationsof new patients
  • Processrepeat prescription requests in accordance with Practice guidelines.

Telephone

  • Useheadset when receiving or making out going calls in accordance with thePractices health and safety guidelines.
  • Answerall calls promptly using the practice script.
  • Dealwith queries as per the practices protocols.

Other Tasks

  • Checksurgeries at the end of the day as per practice protocols.
  • Ensurebuilding security is maintained.
  • Anyother tasks allocated by the Administrator Team and Management Team

  • Inthe course of seeking treatment, patients entrust us with, or allow us togather, sensitive information in relation to their health and othermatters. They do so in confidenceand have the right to expect that staff will respect their privacy and actappropriately
  • Inthe performance of the duties outlined in this Job Description, thepost-holder may have access to confidential information relating topatients and their carers, Practice staff and other healthcareworkers. They may also have accessto information relating to the Practice as a business organisation.All such information from any source is to be regarded as strictlyconfidential
  • Informationrelating to patients, carers, colleagues, other healthcare workers or thebusiness of the Practice may only be divulged to authorised persons inaccordance with the Practice policies and procedures relating toconfidentiality and the protection of personal and sensitive data

The post-holder will assist inpromoting and maintaining their own and others health, safety and security asdefined in the Practice Health & Safety Policy, to include:

  • Usingpersonal security systems within the workplace according to Practiceguidelines
  • Identifyingthe risks involved in work activities and undertaking such activities in away that manages those risks
  • Makingeffective use of training to update knowledge and skills
  • Usingappropriate infection control procedures, maintaining work areas in a tidyand safe way and free from hazards

Person Specification

Qualifications

  • Good educational background
  • A-C grade or equivalent in English and Maths

Experience

  • Experience of working in primary care
  • Experience of working with Systmone
