Social Prescriber - NMWP PCN

We are seeking a compassionate and motivated Social Prescriber to join our Primary Care Network. The role focuses on supporting people to improve their health and wellbeing by addressing non-medical needs and connecting them with appropriate community, voluntary, and statutory services.

The role is part time working 15 hours per week and will be based within GP practices across the PCN (hybrid/community based working). Working closely with GP practices and the wider multidisciplinary team, you will empower individuals to take control of their wellbeing and reduce health inequalities across our local population.

Main duties of the job:

• Work with individuals on a one-to-one basis to identify what matters to them and co-produce personalised support plans.
• Provide holistic support addressing social, emotional, and practical needs such as loneliness, housing, debt, employment, and lifestyle.
• Connect individuals to appropriate community groups, voluntary organisations, and local services.
• Build strong relationships with GP practices, community partners, and local stakeholders.
• Maintain accurate and timely records in line with PCN and NHS requirements.
• Contribute to service development, evaluation, and reporting.
• Work in line with safeguarding, confidentiality, and information governance policies.

About us:

Kingston GP Chambers is a Federation of all NHS General Practices within the Royal Borough of Kingston upon Thames. It was founded in 2008 as a limited company whose shareholders are all 20 GP Practices within Kingston. The company is led by a Board of Directors who are also GP Partners in their own Practices within Kingston.

Kingston GP Chambers has worked collaboratively with both secondary care and the private sector. At present we provide community clinics for Dermatology, Urology, Neurology, Dementia and Diabetes. In addition, Kingston GP Chambers are at the forefront of improving patient access to primary care. We deliver a comprehensive extended hours service offering GP appointments across three hubs in the evenings Monday to Friday, over the weekend and on Bank Holidays. Kingston GP Chambers has the ability to respond rapidly, and we have continued to demonstrate the capability to provide high-quality, innovative and timely solutions to improving patient care and experience. We work closely alongside the other GP Federations in Southwest London to continue the government's five-year forward plan and deliver patient services in the community. We aim to continue to grow as we are increasingly recognised as the provider of choice for extended, enhanced and urgent GP services. We also expect an increasing role as we support, reinforce and develop the position of general practice as the core building block for an increasing range of jointly-provided services.

Qualifications:

• Experience working in a social prescribing, community support, health, social care or voluntary role.
• Strong communication and interpersonal skills.
• Ability to work independently and manage a varied caseload.
• Knowledge of community and voluntary sector services.
• Commitment to person-centred, strengths-based approaches.
• Understanding of safeguarding and confidentiality.
• Social Prescribing qualification or relevant training.
• Experience working within primary care or the NHS.
• Knowledge of local community assets and support services.
• Experience supporting people with complex or multiple needs.
