Surbiton and Chessington PCN have an exciting opportunity for an experienced Clinical Pharmacist to join our dynamic multidisciplinary team. This patient-facing role is to enhance the quality of care for our patients. The successful candidate will be provided with excellent clinical support and an opportunity to develop their primary care skills. We are looking for motivated, team-orientated individuals who share our enthusiasm for improving patient care. Previous primary care experience (ideally a minimum of two years) is preferred as well as excellent verbal communication skills, a proactive approach and a desire to learn.

As the role develops, you will take responsibility for areas of chronic disease management and undertake clinical medication reviews to proactively manage patients with complex poly-pharmacy. You will also provide primary support to general practice staff with regards to prescription and medication queries. The post-holder will be responsible for providing support to the three practices within Surbiton Health Centre (Central, Langley and Brunswick).

Main duties of the job:
  • The post holder will work as part of a multidisciplinary team to provide expertise in clinical medicines management.
  • Provide face to face structured medication reviews.
  • Manage long term conditions, management of medicines on transfer of care and systems for safer prescribing.
  • Manage repeat prescription authorisation and reauthorisations.
  • Patient facing long term condition clinics.
  • Patient facing Clinical and Care Home medication reviews.
  • Management of medications.
  • Unplanned Hospital Admissions.
  • Service Development.
  • Information Management.
  • Provide education and training to Primary Healthcare.

About us: The Surbiton & Chessington PCNs Ltd includes Surbiton Health Centre PCN and Chessington & Surbiton PCN, which work together through a limited company with a joint Board: Surbiton & Chessington PCNs Limited. The PCNs have 3- and 5-member GP practices respectively, and together account for a total registered population of close to 66,000. The networks have diverse communities including commuter areas, young families, deprived pockets, and frail elderly / care homes. The PCNs vision is to help to maintain and improve the health and healthcare for our local populations, and the sustainability of our member GP practices, in line with wider NHS strategies and plans.

The PCN are very flexible with a number of sessions across sites within the borough of Surbiton. Working hours/days are also flexible though some Saturday work will be required. A competitive salary is on offer along with excellent benefits and training and development.

Job responsibilities:
  • Patient facing long term condition clinics.
  • Carry out consultations with patients with single or multiple medical conditions where medicine optimisation is preferred.
  • Review the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicine-taking ensuring they get the best use of their medicines.
  • Make appropriate recommendations to GPs for medicines improvement.
  • Patient facing clinical medication review.
  • Undertake clinical medication reviews with patients and produce recommendations for GPs on prescribing/monitoring.
  • Patient facing Care Home Medication Review.
  • Work with care home staff to improve safety and appropriate medication ordering (medication waste).
  • Patient facing domiciliary clinical medication review.
  • Attend and refer patients to multidisciplinary case conference.
  • Management of common/minor/self-limiting ailments.
  • Telephone medication support.
  • Management of medications at discharge from hospital.
  • Medicines information to practice staff and patients.
  • Unplanned hospital admissions.
  • Repeat Prescribing.
  • Risk stratification.

Person Specification:
  • Mandatory registration with General Pharmaceutical Council (GPhC).
  • Independent prescriber or working towards/intent of gaining an independent prescribing qualification.
  • Completion of CPPE.
  • Member of the Royal Pharmaceutical Society; Hospital or Primary Care experience.
  • In-depth therapeutic and clinical knowledge and understanding of the principles of evidence based Primary healthcare.
  • Minimum of 2 years experience as a pharmacist.
  • Good IT Skills.
  • Good written and oral communication skills.
  • Understanding of the nature of GP and General Practice.
Knowledge and Skills:
  • Understands and demonstrates the characteristics of a role model to members in the team and/or service.
  • Demonstrates self-development through continuous professional development activity.
  • Demonstrates an understanding of current educational policies relevant to working areas of practice and keeps up to date with relevant clinical practice.
  • Ensures appropriate clinical supervision is in place to support development.
  • Demonstrate understanding of the pharmacy role in governance.
  • Demonstrates ability to improve quality within limitations of service.
  • Promotes diversity and equality in people management techniques and leads by example.
