Surbiton and Chessington PCN have an exciting opportunity for an experienced Clinical Pharmacist to join our dynamic multidisciplinary team. This patient-facing role is to enhance the quality of care for our patients. The successful candidate will be provided with excellent clinical support and an opportunity to develop their primary care skills. We are looking for a motivated, team-orientated individual with enthusiasm for improving patient care. Previous primary care experience (ideally a minimum of two years) is preferred, as well as excellent verbal communication skills, a proactive approach and a desire to learn.
As the role develops, you will take responsibility for areas of chronic disease management and undertake clinical medication reviews to proactively manage patients with complex poly-pharmacy. You will also provide primary support to general practice staff with regards to prescription and medication queries. The post-holder will be responsible for providing support to the three practices within Surbiton Health Centre (Central, Langley and Brunswick).
Main duties:
- Work as part of a multidisciplinary team to provide clinical medicines management expertise.
- Conduct face-to-face structured medication reviews and manage medicines on transfer of care, long-term conditions, and safer prescribing systems.
- Manage repeat prescription authorisation and reauthorisations.
- Provide education and training to Primary Healthcare teams.
Job responsibilities:
- Patient facing long term condition clinics: carry out consultations for patients with one or multiple conditions, optimise medicines, review ongoing need and monitoring requirements.
- Patient facing clinical medication reviews: undertake reviews and produce recommendations for GPs on prescribing and monitoring.
- Patient facing care home medication reviews: undertake reviews and work with care home staff to improve safety and appropriate medication ordering.
- Patient facing domiciliary clinical medication reviews: undertake reviews and escalate to senior clinicians as needed; attend multidisciplinary case conferences.
- Management of common/minor/self-limiting ailments: manage caseloads within scope of practice; signpost to community pharmacy and other healthcare professionals; provide telephone support; reconcile medications post-discharge.
- Repeat prescribing and monitoring: adhere to network policies, review requests, and ensure appropriate monitoring tests are in place.
- Service development, information management, medicines quality improvement and medicines safety: contribute to new services, analyse medicines data, undertake clinical audits, implement safety changes and comply with formulary guidance.
- Education and training: provide education on therapeutic and medicines optimisation to primary healthcare teams.
- Public health and CQC: support public health campaigns and ensure CQC standards related to medicines are met.
Qualifications:
- Essential: Masters degree in Pharmacy (MPharm) or equivalent. Mandatory registration with General Pharmaceutical Council (GPhC).
- Desirable: Independent prescriber or working towards/willing to gain independent prescribing qualification. Completion of CPPE; member of the Royal Pharmaceutical Society; hospital or primary care experience. In-depth therapeutic and clinical knowledge of primary healthcare principles.
Experience:
- Essential: Minimum of 2 years experience as a pharmacist. Experience with common acute and long-term conditions in general practice. Ability to identify priorities, refer to seniors/GPs when appropriate, and follow policies and codes of conduct.
- Desirable: Good IT Skills; ability to work under pressure; strong written and oral communication; understanding of GP practice dynamics.
Knowledge and Skills:
- Essential: Demonstrates role-model characteristics and commitment to continuous professional development. Understands educational policies relevant to practice and keeps up to date with clinical practice. Ensures clinical supervision and governance in the workplace. Contributes to the workplace vision and quality improvements within service constraints. Promotes diversity and equality in people management techniques.
- Desirable: Experience with using EMIS.
