We are seeking a compassionate and motivated Social Prescriber to join our Primary Care Network. The role focuses on supporting people to improve their health and wellbeing by addressing non-medical needs and connecting them with appropriate community, voluntary, and statutory services. The role is part time working 15 hours per week and will be based within GP practices across the PCN (hybrid/community based working). Working closely with GP practices and the wider multidisciplinary team, you will empower individuals to take control of their wellbeing and reduce health inequalities across our local population.

Responsibilities

  • Work with individuals on a one-to-one basis to identify what matters to them and co-produce personalised support plans.
  • Provide holistic support addressing social, emotional, and practical needs such as loneliness, housing, debt, employment, and lifestyle.
  • Connect individuals to appropriate community groups, voluntary organisations, and local services.
  • Build strong relationships with GP practices, community partners, and local stakeholders.
  • Maintain accurate and timely records in line with PCN and NHS requirements.
  • Contribute to service development, evaluation, and reporting.
  • Work in line with safeguarding, confidentiality, and information governance policies.
