KCHC is structured to support sustainable primary care practice. Physicians work within an interdisciplinary team and are provided with dedicated time for documentation, care coordination, and complex care planning. The model allows comprehensive care without rushed visits.
The physician works in collaboration with nurse practitioners, social workers, dietitians, diabetic educators, nurses, practical assistance and other allied health professionals to support the health and wellbeing of patients and the community. Members of the inter-disciplinary team assist with forms, non-medical needs, and allied health support around social complexities.
Physicians join KCHC to continue practicing full-scope family medicine. Within this position, you will hold cross-appointments with Queen’s University as Adjunct Clinical Faculty and participate in teaching. This environment allows physicians to practice the full scope of comprehensive family medicine while maintaining professional sustainability.
In this role you will provide comprehensive primary health care services to individuals and families as part of an interdisciplinary team. Care includes health promotion, disease prevention, full life cycle, chronic disease management, and coordination of care with community partners.
