The post holder provides specialist occupational therapy service to clients within Elderly Care Unit. They will be leading the Acute Elderly care OT service and team. The post holder is responsible for managing a caseload of clients with complex needs, using evidence-based and client-centred practice in the assessment and the planning, implementation and evaluation of interventions. The post holder will be working closely with other members of the multidisciplinary team, taking on an active role in identifying, coordinating and organising the appropriate care and rehabilitation service and home/hospital set-up needed to ensure the safe and timely discharge of patient from the acute hospital., The post holder will be providing specialist occupational therapy service to clients within the Elderly Care Unit and leading the elderly care team within OT. -The post holder is responsible for managing a caseload of clients with complex needs, using evidence-basedand client-centred practice in the assessment and the planning, implementation and evaluation of interventions.
