Kings Medical Practice has a great opportunity to join our forward-thinking training practice in Normanton, just off the M62 corridor. Our large modern building was designed to host all services at one site for the benefit of patients and staff, including 24 fully air-conditioned consulting rooms, an operating theatre, and an education suite.

We are focused on promoting work-life balance for our staff with a 90% reduction in administrative burden due to innovative role development and unique technological investment. Our extensive pharmacy team leads on all medication activity, including acute and repeat prescribing, reconciliation of medication, SMRs, promoting eRD, and all quality initiatives.

We have a dedicated multidisciplinary team of ANPs, PAs, and specialist nurses supporting patient care. The culture of training is a core identity of the Practice, and development is actively encouraged with a strong ethos of growing our own.

Main duties of the job:

  • 16 hours per week
  • Patches fully embedded into the ledger
  • Protected admin time

Job responsibilities:

Purpose: To work as part of a multidisciplinary team to enable service delivery and management of high-quality evidence-based care to meet the health and social care needs of the practice population.

Job Summary:

  • Assess, plan, develop, implement, and evaluate treatment programmes that promote health and well-being.
  • Assess, plan, implement, and evaluate individual treatment plans for patients with diabetes.
  • Proactively identify, diagnose, and manage treatment plans for patients with diabetes.
  • Work with other health care professionals to diagnose, monitor, manage, and treat diabetes.
  • Review medication for therapeutic effectiveness, appropriate to patient needs.
  • Support patients to adopt health promotion strategies.
  • Recognise, assess, and refer patients presenting with mental health problems.

Communication:

  • Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment.
  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication.
  • Act as an advocate when representing patients and colleagues.

Delivering a quality service:

  • Produce accurate, contemporaneous, and complete records of patient consultation.
  • Participate in the maintenance of quality governance systems and processes across the practice.
  • Collaborate on improving the quality of health care in partnership with other clinical teams.

Learning and development:

  • Act as mentor to students, assessing competence against set standards.
  • Disseminate learning and information gained to other team members.
  • Assess own learning needs and undertake learning as appropriate.

  • Identify patterns of discrimination and take action to overcome this.
  • Support people who need assistance in exercising their rights.

Person Specification:

  • Recent primary and community nursing experience.
  • Experience of nurse-led management of diabetes patients.
  • Knowledge of needs of patients with diabetes.
  • Registered first level nurse.
  • Independent/supplementary nurse prescribing qualification.
