As a Paediatric Clinical Care Nurse Specialist, you will manage a caseload of patients with palliative care needs and participate in providing a service ensuring patients and their families are fully informed and supported.
This opportunity requires a confident, pioneering, dynamic children\’s nurse to assist in service development and adhere to evidence-based clinical nursing practice in line with current research and guidelines.
The successful post-holder will build on an existing relationship with the Evelina Paediatric Palliative Care team to link with other palliative care providers and primary care, facilitating the care of this group of people in the community.
Responsibilities include:
- Providing specialist nursing services for babies, children, and young people with palliative care needs.
- Offering expert nursing advice and support to patients, families, and healthcare professionals from diagnosis through treatment.
- Assessing health and nursing needs, identifying appropriate care steps, and participating in specialist clinics and MDT meetings.
- Working with the MDT to develop and evaluate care pathways, including end-of-life care.
- Providing emotional support and collaborating with psychology teams.
- Attending outpatient clinics and ensuring patients receive appropriate follow-up and information for informed decision-making.
- Supporting children, young people, and families in participating in care decisions by providing relevant information.
- Liaising with community services, schools, social services, and voluntary organizations to enhance care provision.
- Promoting child well-being and safeguarding, working within policies for reporting concerns.
- Ensuring care is delivered according to protocols, supporting colleagues, and contributing to policy development.
- Coordinating MDT meetings, discharge planning, and developing patient directives.
- Producing accessible information and educational materials for patients and families.
- Engaging in risk management, incident reporting, and audit activities.
- Utilizing information systems efficiently and managing resources within budget.
- Recognizing ethical and legal issues in nursing practice and involving service users in feedback and improvement.
