Locum Consultant General and Liver Intensive Care Medicine

This is a locum critical care consultant post offering an opportunity for excellent individuals to join the critical care team. We are looking for motivated colleagues to help us shape our future and work within the multi–disciplinary Critical Care team across General Intensive Care (GICU), Outreach and Liver Intensive care (LITU), providing high quality care for critically ill patients.

The aim of this post is to provide fixed term consultant cover to King’s Critical Care: adult critical care services at the Denmark Hill site. The post holder will join a large team of Intensivists within the Trust to ensure expert critical care provision on a rotational basis. The current job-plan is annualised over a 42-week rota and will expect the post-holder to provide cover across critical care on a flexible basis. The Critical Care areas to be covered are outlined in a separate document available on request.

The post holder will be required to undertake periods of resident and non-resident critical care duties.

Main duties of the job

The post is based on a 9:1 Direct Clinical Care (DCC) to Supporting Professional Activities (SPA) job plan. The SPA allocation is intended to support continuing professional development, appraisal, revalidation and mandatory professional requirements associated with consultant practice. Further details regarding the allocation of Direct Clinical Care (DCC) Programmed Activities and an indicative job plan can be provided upon request.

Clinical Responsibilities

• Provide consultant-led clinical care for critically ill adult patients across GICU, LITU and the iMobile (critical care outreach) service, ensuring safe, effective and evidence-based management.
• Assess, diagnose and manage patients with complex multi-organ failure, critical illness and acute deterioration requiring intensive care support.
• Lead daily multidisciplinary ward rounds and contribute to the development, review and implementation of individualised treatment plans.
• Deliver timely senior decision-making regarding admission to, discharge from and transfer between critical care services.
• Provide specialist critical care advice and support to inpatient, emergency and specialist teams across the Trust.
• Support the assessment and management of deteriorating patients and second opinions through the iMobile service, facilitating early intervention and escalation of care where appropriate.
• Contribute to consultant-delivered emergency and out-of-hours critical care services, including participation in the consultant on-call rota.
• Support end‑of‑life care, treatment escalation planning and discussions with patients and families regarding goals of care and complex clinical decision‑making.
• Maintain accurate clinical documentation and ensure compliance with national standards, professional guidance and Trust policies relating to critical care practice.
• Provide clinical supervision to resident doctors, ACCPs, nurses and AHPs.
• Offer professional advice and support to multidisciplinary team members.

Continuing Professional Development and Revalidation

• Maintain professional knowledge and clinical competence through participation in continuing professional development activities relevant to critical care medicine and associated specialist services.
• Engage in annual appraisal processes and maintain a portfolio of evidence to support GMC revalidation requirements.
• Maintain awareness of developments in intensive care medicine, liver critical care, patient safety and healthcare policy to support high-quality patient care.
• Contribute to reflective practice and identify personal development objectives aligned to individual, departmental and organisational priorities.
• Participate in mandatory training and statutory requirements necessary to maintain professional registration and clinical practice.

General

The post holder has an important responsibility for and contribution to infection control and must be familiar with the infection control and hygiene procedures and requirements when in clinical areas. These requirements are set out in the National Code of Practice on Infection Control and in local policies and procedures which will be made clear during your induction and subsequent refresher training. These standards must be strictly complied with at all times.

Person specification

• Qualifications: FICM or equivalent e.g. EDIC, FRCA, MRCP, MRCEM or equivalent; Other higher degree or diploma (e.g., MD, PhD, MSc).
• Registration: Full Registration with GMC; Name on GMC Specialist Register on date of taking up appointment.
• Training and Experience: Wide experience in all aspects of Critical Care (including the management of polytrauma, acute neurology, neurosurgical cardiothoracic and haematological emergencies); Practical skill set as appropriate for Critical Care medicine (bronchoscopy, tracheostomy, echocardiography etc.); Extended experience (> 12 months) in advanced Liver critical care and Liver Transplantation; ALS accredited; ATLS or APLS accredited; Management of Difficult Airway; Anaesthesia in remote environments; FUSIC and FICE accredited; Experience with obstetric emergencies; Experience of transfer medicine; Experience with Adolescent medicine.
• Management and Leadership: Able to demonstrate leadership capability within multi-disciplinary teams; Management course and/or qualification; Experience of development of clinical guidelines.
• Audit and Quality Improvement: Thorough understanding of principles of medical audit and quality improvement. Must have undertaken and completed and supervised audit projects and able to demonstrate ability to lead change; Audit/QI Publications; Organisation of Audit/Journal Clubs and Morbidity & Mortality Meetings.
• Research and Publications: Experience of conducting or supporting clinical research; Publication of relevant review articles or case reports; Evidence of research in liver disease, liver critical care or liver transplantation; Post graduate formal research qualification; Evidence of research involving large data or artificial intelligence.
